PDF Trabajo de parto pretérmino es definido como la presencia de actividad uterina asociada a cambios cervicales entre las semanas 20 y 37 de gestación siendo responsable del 70% de la mortalidad perinatal y el 75% de la morbilidad Puede ser clasificado en aquel que debido a determinadas condiciones maternas o fetales indican la interrupción del embarazo, lo cual ocurre en un 30-35% de los casos La amenaza de parto pretérmino puede tener un origen multifactorial. En la mayoría de casos, desconocemos la causa desencadenante. En otras ocasiones, podemos identificar focalidad infecciosa de otras partes del organismo. Pero la causa conocida más frecuente es la infección/inflamación intraamniótica subclínica presente, según datos del Hospital Clínica y de forma muy similar a lo reportado en la literatura, en un 18% del total de mujeres que ingresan por amenaza de parto prematuro. El propósito general de esta investigación será determinar los factores de riesgo que pueden afectar a las mujeres embrazadas de nuestra población y mediante que estrategias se podría disminuir los números de casos El proceso causal del PPT responde a un modelo multifactorial, que determina la activación del trabajo de parto antes de que el producto alcance su madurez in útero. La evidencia científica afirma que la patogenia está liderada por cuatro procesos que tienen como vía final, el inicio de las contracciones uterinas y la consiguiente dilatación y borramiento del cérvix uterino. Estas vías patogénicas son: a. Activación del eje hipotálamo-hipófisis-adrenal materno o fetal, b. Inflamación local (decidual o corioamniótica) o sistémica, c. Hemorragia decidual (abruptio placentae), d. Distensión patológica del útero. En lo referente a factores de riesgo para PPT, se han descrito multiplicidad de variables, las cuales generalmente interactúan entre sí. Se describe que los factores más constantes relacionados con el PPT son el stress psicosocial y la infección materna, otros factores citados en la literatura incluyen: el cambio de pareja entre el 8 primero y segundo embarazo, el periodo intergenésico corto (menor de 6 meses) que duplica el riesgo de PPT, trastornos alimentarios previos al embarazo, el uso de antidepresivos tricíclicos durante el tercer trimestre del embarazo, el antecedente de PPT, bajo peso materno y/o baja ganancia de peso durante el embarazo, el habito de fumar, embarazo múltiple y sexo masculino en ambos gemelos, embarazos concebidos por fertilización in Vitro, antecedente de aborto espontáneo o inducido, rotura prematura de membranas. The causal process of the PPT responds to a multifactorial model, which determines the activation of work of labor before the product reaches maturity in utero. The scientific evidence affirms that the pathogenesis is led by four processes that have as final way, the beginning of the uterine contractions and the consequent dilatation and erasure of the uterine cervix. These pathogenic pathways are: a. Activation of the hypothalamic-pituitary-adrenal maternal or fetal axis, b. Local inflammation (decidual or chorioamniotic) or systemic, c. Decline hemorrhage (placental abruption), d. Pathological distention of the uterus. Regarding risk factors for PPT, a multiplicity of variables have been described, which generally interact with each other. It is described that the most constant factors related to the PPT are psychosocial stress and maternal infection, other factors cited in the literature include: the change of couple between the 8 first and second pregnancy, the short intergenic period (less than 6 months) that doubles the risk of PPT, eating disorders prior to pregnancy, the use of tricyclic antidepressants during the third trimester of pregnancy, the history of PPT, low maternal weight and / or low weight gain during pregnancy, smoking, multiple pregnancy and male sex in both twins, pregnancies conceived by in vitro fertilization, history of spontaneous or induced abortion, premature rupture of membranes.
