PDF El hematoma epidural es la colección hemática en el espacio epidural (entre la tabla interna ósea y la duramadre) generalmente secundaria a una lesión traumática. Es una patología con altas tasas de morbimortalidad en el mundo entero. Las manifestaciones clínicas más frecuentes son la cefalea, perdida de la conciencia, vómitos y convulsiones; aunque pueden presentarse con síntomas muy leves que para los médicos con poca pericia pueden ser imperceptibles de ahí la importancia del conocimiento de esta entidad. Su origen habitual es el tipo venoso y las localizaciones más frecuentes unilaterales en el 95% de los casos, y a nivel lobar la mayor representación es la temporoparietal. El diagnóstico está dado por el antecedente del trauma, la sintomatología y signología y se confirma con la tomografía axial computarizada que es el gold estándar en el trauma craneoencefálico. El tratamiento puede ser conservador y/o neuroquirúrgico en dependencia del cuadro clínico, el volumen y la localización del hematoma. De un total de 480 pacientes evaluados en el Hospital Docente de Especialidades Guayaquil Dr. Abel Gilbert Pontón con un diagnóstico inicial de traumatismo craneoencefálico el 3,9% corresponde a pacientes con un hematoma epidural. The epidural hematoma is the hematic collection in the epidural space (between the internal bone table and the dura) usually secondary to a traumatic injury. It is a pathology with high rates of morbidity and mortality in the entire world. The most frequent clinical manifestations are headache, loss of consciousness, vomiting and seizures; although they may present with very mild symptoms that for physicians with little expertise may be imperceptible hence the importance of knowledge of this entity. Its usual origin is the venous type and the most frequent unilateral locations in 95% of the cases, and at the lobar level the greatest representation is the temporoparietal. The diagnosis is given by the antecedent of the trauma, the symptomatology and the signology and it is confirmed with the computerized axial tomography that is the gold standard in the cranioencephalic trauma. The treatment can be conservative and / or neurosurgical depending on the clinical picture, volume and location of the hematoma. Of a total of 480 patients evaluated in the Teaching Hospital of Guayaquil Specialties Dr. Abel Gilbert Pontón with an initial diagnosis of traumatic brain injury, 3.9% corresponds to patients with an epidural hematoma.
