PDF RESUMEN Hace ya más de un siglo se describió la presencia en la superficie mucosa del estómago de bacterias espiraladas. Esto fue interpretado como un fenómeno de contaminación menospreciado por la acción seguramente bactericida del ácido. En un estudio publicado en 1984 en la revista Lancet por Marshall y Warren se destaca la alta incidencia de úlcera gástrica y especialmente duodenal en pacientes colonizados por un bacilo Gram negativo espiralado que primero se clasificó como Campilobacter Pylori y circula hoy en toda la literatura mundial como Helicobacter Pylori. Esta bacteria, adquirida generalmente en la infancia, que se adhiere firmemente al epitelio de la mucosa gástrica puede demostrarse en el 90 al 95 % de los pacientes que sufren úlcera duodenal y en más del 85 % de los que sufren de úlcera gástrica. El Helicobacter pylori debilita el revestimiento mucoso que protege el estómago y el duodeno, lo cual permite que el ácido afecte la superficie sensible que se halla por debajo de dicho revestimiento. Por efecto tanto del ácido como de la bacteria, esa superficie delicada se irrita y se forma una llaga o úlcera. El diagnóstico de la infección por Helicobacter se establece por pruebas de sangre, aliento y tejido. Las pruebas de sangre son las más comunes. Permiten detectar anticuerpos contra dicha bacteria. La prueba del aliento se usa principalmente después del tratamiento para ver si éste dio resultado, pero se puede usar también para el diagnóstico. Las pruebas tisulares se hacen generalmente con el tejido que se extrae con el endoscopio. Las hay de tres tipos: La prueba rápida de ureasa detecta esta enzima, que es producida por H. Pylori. La prueba histológica permite que el médico observe y examine la bacteria en sí misma. La prueba de cultivo permite que el H. Pylori se reproduzca en la muestra de tejido. El tratamiento generalmente entraña la combinación de antibióticos, supresores del ácido y protectores del estómago. En la actualidad la forma más eficaz de tratar el problema consiste en administrar durante dos semanas lo que se conoce como terapia triple, es decir, tomar dos antibióticos para matar la bacteria y bien sea un supresor de la secreción de ácido o un protector del revestimiento gástrico. La terapia triple administrada durante dos semanas disminuye los síntomas ulcerosos, destruye las bacterias y evita la recurrencia de la úlcera en más del 90 % de los pacientes. Para cerciorarse de que el tratamiento ha sido efectivo, hay que realizarse una endoscopia de seguimiento o una prueba del aliento entre 6 y 12 meses después para comprobar si todavía hay bacteria. Existe la ventaja que una vez erradicada, la tasa de reinfección es muy baja. Los investigadores están tratando de obtener una vacuna para prevenir la infección. Aunque con certeza no se conoce cómo se propaga la bacteria, ésta puede ser transmitida de persona a persona mediante agua y alimentos contaminados. Existen pacientes que tienen la bacteria, pero no desarrollan síntomas, permaneciendo asintomáticos toda su vida, mientras que otros, por diversos factores, presentarán enfermedades digestivas más comunes: gastritis, úlcera gástrica y duodenal, dispepsia y en casos más graves, cáncer gástrico. Estos factores son: bajo nivel socioeconómico, condiciones de hacinamiento y deficientes normas de higiene. En nuestro estudio, realizado en consultorio y laboratorio particulares, donde los pacientes que van no pertenecen a un grupo social bajo, vimos que no se cumple la regla de bajo nivel socioeconómico, puesto que la población investigada no pertenece a ese segmento poblacional. Pero en cambio, estas personas, hombres y mujeres adultos, en edades comprendidas entre los 17 a 60 años, que llegaron a consulta con síntomas clínicos, que fueron evaluados endoscópicamente y valorados serológicamente para determinar la presencia de la bacteria en su organismo, se describieron como gente que come en la calle, que no guarda buenos hábitos alimenticios, con trasfondo familiar de enfermedades digestivas y automedicados. Todos esos factores influyeron para que lleguemos a la conclusión que debe haber educación para la población a fin de prevenir enfermedades y aprender a vivir en forma más saludable. The Helicobacter pylori weakens the mucous lining that protects the stomach and the duodeno, that which allows that the acid affects the sensitive surface that is below this lining. For so much effect of the acid as of the bacteria, that delicate surface is irritated and she/he is formed a wound or ulcer. The diagnosis of the infection for Helicobacter settles down for tests of blood, encouragement and fabric. The tests of blood are the most common. They allow to detect antibodies against this bacteria. The test of the encouragement is used mainly after the treatment to see if this gave result, but you can also use for the diagnosis. The tests tisulares is generally made with the fabric that is extracted with the endoscopio. There are them of three types: 1. the quick test of ureasa detects this enzyme that is produced by H. Pylori. 2. the test histológica allows the doctor to observe and examine the bacteria in itself. 3. the cultivation test allows that the H. Pylori reproduces in the fabric sample. The treatment generally involves the combination of antibiotics, suppressive of the acid and protective of the stomach. At the present time the most effective form of treating the problem consists on administering that is to say during two weeks what is known as triple therapy, to take two antibiotics to kill the bacteria and good is a suppressive of the acid secretion or a protector of the gastric lining. The triple therapy administered during two weeks diminishes the ulcerous symptoms, it destroys the bacterias and it avoids the recurrencia of the ulcer in more than 90% of the patients. To make sure that the treatment has been effective, it is necessary to be carried out a pursuit endoscopia or a test of the encouragement later between 6 and 12 months to check if there is still bacteria. It exists the advantage that once eradicated, the reinfección rate is very low. The investigators are trying to obtain a vaccine to prevent the infection. Although with certainty it is not known how the bacteria spreads, this can be transmitted from person to person by means of water and polluted foods. They exist patient that they have the bacteria, but they don't develop symptoms, remaining asintomáticos all their life, while other, for diverse factors, they will present more common digestive illnesses: gastritis, gastric ulcer and duodenal, dyspepsia and in more serious cases, gastric cancer. These factors are: under socioeconomic level, conditions of accumulation and faulty norms of hygiene. In our study, carried out in clinic and laboratory matters, where the patients that go don't belong to a social group under, we saw that the rule of low socioeconomic level is not completed, since the investigated population doesn't belong to that populational segment. But on the other hand, these people, men and women adults, in ages understood among the 17 to 60 years that arrived to consultation with clinical symptoms that endoscópicamente and valued serológicamente were evaluated to determine the presence of the bacteria in their organism, were described as people that eats in the street that doesn't keep good nutritious habits, with family trasfondo of digestive illnesses and automedicados. All those factors influenced so that we reach the conclusion that should have education for the population in order to prevent illnesses and to learn how to live in healthier form. |
