PDF La atresia de vías biliares se caracteriza por la obliteración o la discontinuidad del sistema biliar extrahepático, lo que da como resultado una obstrucción del flujo biliar. Este trastorno representa en el período neonatal la causa más común de colestasis quirúrgica. La patología del sistema biliar extrahepático varía ampliamente en estos pacientes, y la siguiente clasificación se basa en el sitio predominante de atresia ya sea por la obliteración del conducto común, por la atresia del conducto hepático, con estructuras quísticas que se encuentran en la porta, o aquella que implica atresia de los conductos hepáticos derecho e izquierdo al nivel de la porta hepática. A lo largo del desarrollo de esta patología, se produce daño principalmente hepático dando como consecuencia final a una Insuficiencia Hepática Crónica debida a una complicación por Colangitis y/o Cirrosis hepática, con desenlace fatal en el estado crítico del paciente. Biliary tract atresia is characterized by obliteration or discontinuity of the extrahepatic biliary system, which results in an obstruction of the bile flow. This disorder represents in the neonatal period the most common cause of surgical cholestasis. The pathology of the extrahepatic biliary system varies widely in these patients, and the following classification is based on the predominant site of atresia, either due to obliteration of the common duct, atresia of the hepatic duct, with cystic structures found in the portal, or that which involves atresia of the right and left hepatic ducts at the level of the hepatic portal. Throughout the development of this pathology, mainly liver damage occurs resulting in a chronic liver failure due to a complication by cholangitis and/or liver cirrhosis, with fatal outcome in the critical condition of the patient.
