pdf El tejido gingival es uno de los tejidos que se encuentra con mayor frecuencia pigmentado por máculas melanocíticas a nivel vestibular en la región anterior del maxilar superior como del inferior. Estas manchas denominadas pigmentaciones de melanina no representan un factor de riesgo para el ser humano ni tampoco una patología a nivel de la cavidad bucal, pero sí se muestra como un problema estético para el paciente que lo presenta. La melanina es la principal causante de la melanosis gingival asociada por lo general a factores étnicos, administración de medicamentos, el fumar, lesiones vasculares, tatuajes metálicos por amalgama y diversas patologías, de las cuales se deben diferenciar cada una de ellas mediante el estudio histopatológico. El tratamiento del peeling periodontal consiste en la eliminación de las hiperpigmentaciones melánicas que se pueden encontrar en la encía marginal, encía insertada y en la papila interdental vestibular, mediante diversas técnicas como la criocirugía, mucoabrasión, láser, electrocirugía, métodos químicos, aloinjertos y el uso del bisturí manual, siendo esta última la técnica aplicada en la presente investigación por sus múltiples ventajas. El objetivo de este caso clínico es demostrar la eficacia de esta técnica de peeling mediante el uso del bisturí manual presentándola como una alternativa de procedimiento seguro, poco invasiva, eficiente y económica. El caso clínico realizado obtuvo excelentes resultados, ya que se lograron eliminar las pigmentaciones fisiológicas en la paciente y así se pudo mejorar su apariencia estética, llegando a la conclusión de que este método utilizado es eficaz y poco invasivo. The gingival tissue is one of the tissues where you can find pigmentation with more frequency; this type of pigmentation is caused by melanosis stains in the vestibular gingival surface at the gingival frontier of the maxilla and the mandible. These melanosis stains called melanin pigmentations doesn´t represents any risks for the human being, neither an oral pathology, but it is found as an unaesthetic problem for the patient that suffers from this. The melanin is the main cause of the gingival melanosis associated with ethnic factors, the administration of certain medicines, habits such as smoking, vascular lesions, metallic tattoos caused by amalgam and various pathologies, they must differ one from another with the of histopathology diagnosis. The treatment of gingival depigmentation consist in the elimination of the melanin hyperpigmentation that can be found in the marginal gingiva, attached gingiva and the interdental gingiva, by using several techniques such as the cryosurgery, mucoabrasion, laser, electrosurgery, chemical methods, allograft and the use of the scalpel, being this last one option the technique applied in this investigation because of its multiple advantages. The objective of this clinical case is to demonstrate the efficacy of this technique of gingival depigmentation with the usage of the scalpel, presenting it as an alternative of a secure procedure, less invasive, efficient and cheaper than other procedures. This clinical case gave excellent results, because it eliminates the physiological pigmentations in the patient and this improves her esthetic appearance, the conclusion of this method is efficient and less invasive. |
